Pay Review

This form is to be filled in by the store manager of the employees that is requesting a pay amendment:

1. Please ensure you have completed a employee performance appraisal  with the member of staff prior to completing this pay amendment form

2. Fill in this form to the best of your knowlege and include as much  information as possible. The more information you can provide will  assist us in confirming the payrise is warranted

3. When you click submit all the information will be sent to your area manager for their approval.

4. Once a decision has been made you will receive an email informing you of the outcome.
