Please make sure you fill out the below form with as much detail and as accurately as possible. All staff members must be invited to take part in the activity and adequate cover must be found for your store.

This request must be approved BEFORE you book anything.

Once approved you can book your activity and have fun ! Please make sure you take lots of photos for the newsletter!
